La derivación o el baipás biliopancreático, con o sin “llave” duodenal (intervención de Scopinario), hoy en día es la técnica más eficaz para la pérdida de peso o reducción del por ciento de obesidad posterior. Esta técnica consiste en un baipás de una parte del intestino que absorbe la comida consumida. El paciente lleva una vida mejor en comparación a la con anillo gástrico porque los efectos secundarios de las intervenciones de reducción (vómitos, dificultad al tragar etc.) se reducen. Pueden aparecer, sin embargo, sobre todo después de la consumición de grasas o postres, diarreas y crujidos del estómago (borborigmos). Estos pacientes deben tomar vitaminas y alimentos adicionales (vitamina B, ácido fólico), calcio y pastillas hierro toda su vida. Si no lo hacen pueden sufrir una anemia, hiperparatireoidismo segundario u osteoporosis. La otra ventaja se esta técnica es que permite la realización de un examen del resto del estómago mediante un gastroscopio. Durante la intervención no se realizan incisiones porque se puede hacer de manera laparoscópica y con un robot, el paciente queda en el hospital unos 3 ó 4 días. Pueden observarse algunas complicaciones inmediatas (escape anastomosis, hemorragia) que se pueden tratar con los métodos tradicionales o mediante una segunda intervención. El total por ciento de las complicaciones asciende a un 4-6%. El por ciento de los pacientes que quedan satisfechos de los resultados es muy alto (90 – 95%).
Baipás gástrico Roux-en-Y
La principal diferencia entre la intervención anterior y ésta es que el fondo estomacal queda (no se realiza gastrectomía). En la parte inferior del estómago se forma una bolsita que queda totalmente separada del resto del estómago y está unida mediante una anastomosis en parte media del intestino delgado con Roux-en-Y. La modificación de esta técnica y la introducción del duodeno llevan a la conservación del píloro y a una disminución considerable de las complicaciones ya que se reducen los síntomas del síndrome de dumping agudo y de las úlceras anastómicas.
La intervención lleva a una mayor y más duradera pérdida de peso (al menos un promedio de un 65 % de pérdida d peso por 5 años o más). Habitualmente conlleva la colecistectomía. Desde el punto de vista técnico, esta intervención es muy complicada y causa más problemas metabólicos en comparación con las técnicas arriba mencionadas.
